Upper and Lower Blepharoplasty: Refreshing Your Gaze While Keeping Your Identity

The passage of time, genetics, or simply the natural anatomy of our face can lead to a progressively heavy and tired-looking gaze. Excess skin on the upper eyelids and prominent bags under the eyes are among the most common aesthetic concerns, often making the face appear perpetually fatigued. In the clinical case we are looking at today, the patient wanted to address these exact issues to restore a fresh, vibrant look to her face.

During the pre-operative consultation, the patient highlighted two main concerns. Firstly, the noticeable under-eye bags and dark circles that dulled her expression. Secondly, the sagging skin on her upper eyelids, which not only made her eyes look “closed” but also created a practical daily frustration: applying makeup, such as eyeliner, had become difficult because the product would simply get lost within the folds of the drooping skin.
To address these needs with a comprehensive and harmonious approach, I recommended a combined procedure. We performed an upper blepharoplasty to remove the excess eyelid skin (carefully preserving unique facial features, like a mole the patient wished to keep) alongside a lower blepharoplasty to permanently eliminate the under-eye bags.

The video showcases the patient just one month after the surgery. The results speak for themselves: the scars are already virtually invisible. Her gaze is open, lightened, particularly on the lateral sides, providing a beautiful, natural lifting effect.

The most crucial aspect of this transformation—and the greatest satisfaction for an oculoplastic surgeon—is the patient’s reaction when asked, “But is it still your eye?” Her response, “Yes, it’s still my eye, but it’s cleaner, fresher,” captures the very essence of our aesthetic philosophy. The goal is never to distort or change who you are, but to reveal the best, most rested version of yourself.
