Understanding Upper Blepharoplasty: Variations and Techniques

When they tell me that upper blepharoplasty is a simple procedure, upper blepharoplasty is not always the same; it varies quite a bit. It is a very delicate and intricate procedure that can change the shape of the gaze in carefully selected patients. Not all upper eyelids are the same; we have two main and distinct categories. The typical deep and well-defined eyelid, with a large distance between the eyebrow and upper eyelids, is a Western uncovered eyelid. In that case, it is the classic upper blepharoplasty, where you can make a high incision, remove a lot of tissue, and eliminate excess skin. This is the standard upper blepharoplasty procedure.

But the other variant is the oriental eye, which has an elongated, horizontal, and hooded shape. In these cases, blepharoplasty, if done as in the previous case, requires careful consideration. How can we preserve the shape of the hooded oriental eye while transforming a covered eyelid into a round eye? By removing the excess skin, which still causes heaviness, we can achieve a low, horizontal eyelid fold by carefully cutting near the lashes while maintaining the shape intact. Maintain this precise cut for the desired effect. Clean more skin on the sides than in the middle, in such a way as to keep a small amount of mobile eyelid, ensuring it stays balanced. This allows just enough space to apply eyeliner and fully see the lashes, but not reveal too much of the mobile eyelid. Otherwise, it becomes a round eye, which is not pleasing to look at. Therefore, each patient’s upper blepharoplasty is a one-of-a-kind procedure, tailored to their individual expectations, requirements, and eye contact.
