Reaching the end of the day with tired, heavy eyes—feeling as if your eyelids are closing on their own—is a common complaint, particularly for those who spend long hours working on a computer. This isn’t just an aesthetic concern regarding a fatigued appearance; it’s a genuine functional issue. In today’s clinical case, we explore the journey of a patient whose primary goal was to refresh her spirit, brighten her look, and finally appear energized and rested.
During the consultation, we identified two main issues contributing to her tired appearance. First, there was significant excess skin on the upper eyelid that folded down and rested directly on her eyelashes, causing functional heaviness. Second, there was a noticeable presence of fat pads (bags) and hollow grooves around the lower orbit. When she smiled, the thin skin gathered over these fat pads, making the swelling look even more pronounced. To address this comprehensively, we decided to perform both an Upper Blepharoplasty and a Lower Blepharoplasty.
The surgical technique was highly customized. For the lower eyelids, rather than simply removing the fat, we utilized the existing medial, central, and lateral fat pads to fill in the hollows around the orbit. We then carefully lifted the skin upwards with a specific eyelid lift maneuver. A critical aspect of this technique is that it rejuvenates the under-eye area without altering the natural shape of the eye whatsoever, as no canthopexy or canthoplasty was performed.
At just the one-month post-operative mark, the surgical marks are already minimal, and the aesthetic result is beautifully clean. However, the most profound impact for the patient has been the wonderful feeling of lightness in the evenings. Working all day at a screen no longer results in a drooping eyelid that forces the eye shut.
