Seeing the World Again at 70: A Complex Case of Hereditary Myopathic Ptosis

Imagine having to tilt your head back constantly or use your fingers to physically lift your eyelids just to watch television or see the face of the person in front of you. This was the daily reality for a 70-year-old patient of ours from Sardinia. For over two decades, he struggled with a severe form of hereditary myopathic ptosis—a debilitating condition that reduced his vision to a blurry, foggy world plagued by double vision.


Myopathic ptosis is fundamentally different from the more common age-related (aponeurotic) droopy eyelids. In this specific condition, the issue lies within the muscle tissue itself. The levator muscle is structurally weak or dysfunctional due to genetic factors passed down through the patient’s family. Because the primary muscle is compromised, traditional eyelid surgery often yields insufficient results, which is why a previous surgery performed by another doctor had failed to correct his vision.

To resolve such complex clinical challenges, specialized oculoplastic surgery utilizes a different mechanical approach. In this case, we performed a frontalis suspension procedure. Essentially, we established a direct physical connection between the drooping eyelid and the eyebrow. This technique allows the patient to effectively open his eyes simply by raising his eyebrows, completely bypassing the weakened eyelid muscle.

The results, seen at his 13-day post-operative checkup, are profoundly moving. The patient can now open and close his eyes naturally and completely, without any discomfort. However, the true success lies in his restored quality of life. He described seeing clear colors and beautiful light again, free from the “fog” and double vision that had plagued him for twenty years. He even celebrated his first Christmas with his eyes fully open, rating his happiness at a “9.95 out of 10.”
