Correcting Eye Asymmetry and Dark Circles in Young Male Patients

Eye asymmetry is a very common facial characteristic, but in some instances, it can significantly impact overall facial harmony. Many patients, even from a young age, notice distinct differences between their right and left eyes, often accompanied by a persistently tired appearance. In the clinical case we are examining today, the young male patient presented with a combination of aesthetic concerns primarily affecting his left eye: a drooping upper eyelid, a retracted lower eyelid exposing the white of the eye, and pronounced dark circles.


Addressing such a multifaceted clinical scenario requires a customized surgical approach aimed at restoring symmetry and rejuvenating the eye area without altering the patient’s natural features. The first step involved correcting the upper eyelid ptosis. To lift and open the eyelid, we utilized an internal approach. This advanced technique allows us to adjust the underlying muscle without leaving any visible external scars.
The second critical issue was the lower eyelid retraction, clinically known as scleral show. The excessive exposure of the sclera can make the eye look unusually round and protruding. To address this, we performed a canthopexy combined with the release of the lower eyelid retractor. This procedure effectively tightened and repositioned the lower eyelid, with only a minimal, imperceptible scar hidden within the natural crease of the upper eyelid.

Finally, to resolve the tired look caused by the dark circles—characterized by both puffiness and hollowness—we performed a transconjunctival lower blepharoplasty. Operating completely from the inside, without cutting the skin, we repositioned the excess fat from the eye bags to smoothly fill in the hollow areas below.

The outcome of this combined surgery is a complete optimization of the facial asymmetries and a refreshed appearance. The true success of modern oculoplastic surgery lies in its subtlety: achieving a symmetrical, rested, and entirely natural look that leaves no obvious signs of surgical intervention.
