Finding Yourself Again: The Success of Comprehensive Eyelid Surgery at Just 12 Days

When a patient walks into my office stating, “I’m not myself anymore,” my role as an oculoplastic surgeon extends far beyond mere aesthetic correction. The clinical case we are presenting today involves a woman troubled by persistent periocular swelling. This issue had been ongoing for about a year and was significantly exacerbated by previous hyaluronic acid fillers—a substance that frequently retains water, leading to chronic edema in such a delicate area.


Clinical Case Analysis During the consultation, a thorough examination revealed a combination of structural and aesthetic concerns. On the upper eyelid, the patient suffered from loose, heavy skin that was visibly thickened by swelling. Additionally, we identified an early-stage xanthelasma (a yellowish cholesterol deposit) near the inner corner of the eye. The severe skin sagging created a noticeable closure on the lateral side of her vision.

Moving to the lower eyelid, the patient exhibited prominent heavy bags, significant malar edema, and excess loose skin that desperately needed addressing.

The Surgical Approach To restore a fresh, rejuvenated appearance without altering her fundamental facial identity, we opted for a highly targeted approach. We performed an Upper Blepharoplasty to remove the excess skin and expertly excise the xanthelasma, ensuring the incision was perfectly hidden within the natural eyelid crease.

For the lower eyelid, we executed a Lower Blepharoplasty utilizing a fat transposition technique through a discreet subciliary incision. This advanced method allows us to reposition tissues and eliminate bags without creating a hollowed look. We deliberately avoided lifting the brow, focusing exclusively on the eyelid region to guarantee the most natural outcome possible.

A Rapid and Remarkable Recovery Just 12 days post-surgery, during the stitch removal appointment, the results speak for themselves. Despite a very mild, expected residual swelling, the scars are already virtually invisible.

The most profound transformation, however, is emotional. The patient reports feeling like a completely different person. The heavy bags and persistent edema are gone, and the heavy look has been replaced by eyes that are finally smiling again. It is an outstanding result that marks the beginning of a true “new life.”
