Rejuvenating the delicate eye area requires a highly customized and expert approach, particularly when addressing the outcomes of previous cosmetic procedures. In today’s clinical case, we examine a patient who came to us with pronounced under-eye swelling and a significant lateral drooping of her gaze. The patient had previously undergone a traditional facelift combined with lipofilling elsewhere, which over time contributed to the formation of solid fat deposits and an overall heaviness in the periorbital region.
During the comprehensive pre-operative consultation, we identified the critical need for a multi-faceted surgical strategy to restore a fresh, open, and youthful look to her eyes. The primary goal was to open and lift the area, focusing heavily on elevating the lateral tail of the brow and eye. To achieve this harmonious result, we designed a customized three-step surgical plan. First, to beautifully correct the lateral drooping and elevate the midface tissues, we performed an Endoscopic Face Lift. This advanced, minimally invasive technique allows for the precise repositioning of facial structures without extensive scarring.
Next, to successfully address the solid fat deposits and tighten the lax skin under the eyes, a Lower Blepharoplasty was essential. This crucial step smoothed the lower eyelid contour, correcting the swelling exacerbated by her previous lipofilling. Finally, we completed the rejuvenation process with an Upper Blepharoplasty to correct the heavy closing of the gaze, carefully lifting and tightening the excess upper eyelid skin.
Just over a month post-surgery, the patient’s transformation is remarkable. The heavy swelling is completely resolved, the lateral droop is expertly corrected, and her gaze is beautifully bright and open. Complex revisional cases require tailored, combined procedures to restore true facial harmony.
