Scarless Eyelid Ptosis Surgery: How 2 Millimeters Change Your Gaze

Eyelid ptosis is a highly common condition characterized by the drooping of the upper eyelid, which consequently covers a portion of the eye. This anatomical shift does more than just obstruct vision; it profoundly alters facial harmony. Patients with ptosis often present with a chronically tired and asymmetrical appearance. To subconsciously compensate for the drooping lid, they tend to elevate their eyebrow on the affected side, which inappropriately exposes more eyelid skin. In the delicate periorbital area, a structural drop of just two millimeters is enough to create significant and noticeable facial asymmetry.

In the recent clinical case of our patient Chiara, we addressed these exact aesthetic and functional concerns. Our primary goal was to restore a symmetrical, open gaze without leaving any telltale signs of surgery. To achieve this, we utilized a specialized internal surgical approach focusing on Müller’s muscle. By gently everting (flipping) the eyelid, we were able to shorten the muscle from the inside, entirely avoiding external incisions. With this technique there are no external stitches and no skin scar.

The benefits of a scarless approach are most evident during the recovery phase. Just one week post-surgery, Chiara’s eyelid was already in exceptional condition with a rapid, comfortable recovery. At the six-month mark, the change is clear. Her eyes are open, perfectly symmetrical, and the previously elevated eyebrow has settled back into its natural, relaxed position.

The overall outcome is natural, leaving no visible evidence of surgical intervention. The psychological and emotional impact of this procedure is just as important as the physical changes: Chiara was so thrilled with her new look that she immediately felt confident enough to start taking photos again, no longer needing to hide behind her glasses.
