Correcting a Failed Canthopexy: Maurizia’s Revision Journey

Dealing with the aftermath of an unsuccessful surgical procedure is both emotionally and physically exhausting. Maurizia came to our clinic in a state of deep distress: to support her lower eyelid, which had become severely retracted and detached, she was forced to apply an adhesive bandage to her face every single day. This distressing situation had been ongoing for some time, profoundly affecting her self-esteem and making her feel constantly “sad” and uncomfortable with her own reflection.

Maurizia’s clinical issue stemmed from an upper and Lower Blepharoplasty performed years ago, which was followed by a canthopexy that failed to yield the desired results. Despite multiple subsequent touch-ups attempting to fix the issue, the tissue laxity ultimately led to severe eyelid retraction. In complex secondary surgery cases like this, operating solely on the eyelid is insufficient, as the surrounding tissues have lost their structural support.

To resolve this issue, the surgical solution required a comprehensive, minimally invasive approach to redistribute the mid-face volumes. We performed an Endoscopic Face Lift specifically designed to elevate the temporal and malar areas. By lifting the tissues of the mid-face, we successfully relieved the mechanical tension on the lower eyelid, creating the optimal environment to perform a new, definitive canthopexy.

This surgical synergy allowed us to reposition the eye naturally and securely, permanently eliminating her need for external support like the bandage. Today, Maurizia has found her peace of mind again. Restoring not only the eyelid’s function but also the joy of “feeling like herself” is the ultimate goal of our reconstructive approach.
